There are many physicians who advocate for progressive and ethical tobacco control policies. Meetings like GFN provide a venue where we can get recharged and know that we are not alone with our advocacy. This session will provide an open forum to discuss challenges facing physicians in our home countries. While focusing on physician advocacy, the input from non-physician attendees would be welcome. The Tobacco Products Directive (TPD) sets many of the rules that govern how tobacco and nicotine products are made available and accessible to consumers. The latest revision, TPD3, is now underway. This session aims to determine and inform how to support harm reduction in TPD3's development and implementation. What lessons can we take from TPD2? What are the TPD3 processes, the politics, the expected approach to evidence? Is there a role for people who smoke or who use safer nicotine products?

There have been enormous social and cultural impacts on restrictions of safer nicotine products, whether that be advertising, taxes, or other government interventions. Some key issues that arise from this are: illicit markets have exploded and culturally normalised the purchase of nicotine products on the black market; the unintended and counterproductive consequences of prohibitionist policies on society and impact public understanding/opinion: equity implications of prohibition - for example, smoking rates are higher amongst lower socioeconomic groups, and so prohibitionist policies on safer nicotine hurt them the most.

Are multi-national organisations constraining national sovereignty in tobacco harm reduction policy? At the WHO’s COP11 in Geneva, the Secretariat and aligned NGOs pressed delegations to adopt sweeping “forward-looking measures,” including severe restrictions and possible prohibitions on non-combustible nicotine products, potentially limiting countries’ ability to pursue risk-proportionate approaches. Meanwhile, the European Commission’s review of the Tobacco Products Directive begins from the premise that safer nicotine products are as harmful as smoking and could introduce binding, disproportionate restrictions across all 27 Member States. This panel examines recent developments at global and European levels.

Harm reduction approaches are now an established and fundamental part of the response to drug use. While tobacco harm reduction has an expanding evidence base it is often viewed in narrow terms, as if people who smoke exist in a discrete silo. This has meant the potential of THR to help communities with high smoking prevalence, such as people who use drugs, remains underexplored – despite the evident harms they experience and their impact on health outcomes. This session brings a drug harm reduction perspective to THR, drawing on frontline experience across a range of environments and sectors, seeking to highlight what works in terms of harm reduction for individuals and communities, plus opportunities for further development.

More than a billion adults globally smoke combustible cigarettes. The harm caused can be reduced if individuals switch completely to smoke-free products. Availability of a wide range of options can accelerate their switching journey. However, access to products is being restricted by either outright bans or flavour restrictions. Whilst aimed at preventing underage use, this overlooks the needs of adults who smoke. This session will focus on a rigorous, evidence-based evaluation of whether product/flavour bans achieve the objective of reducing youth use of smoke-free products, without undermining adult smokers’ access to effective switching options.

The panel will discuss the various forms of prohibition, examine the main objections to prohibition, and ask under what circumstances, if any, prohibitions are justified. We consider the likely consequences of prohibition, especially in the counterintuitive case where prohibition applies to products safer than the market norm. What are the merits of two common strategies for addressing the costs and consequences: a stronger enforcement approach or removing the underlying economic incentives? We will conclude with an engaging finale, in which the panel and audience are asked to provide unwanted advice to the Australian government, Michael Bloomberg and the WHO.
